Since the usual cause of a pinched nerve is a herniated disc, it is essential to reduce the disc herniation to get pressure off the nerve root as quickly as possible. Unfortunately, the usual treatment is an injection to reduce the inflammatory consequence of the disc herniation. Since inflammation includes four components, i.e. redness, swelling, heat and pain, and since injections reduce inflammation, the injection will reduce swelling, but will it help the swelling enough to reduce pressure on the nerve? Maybe, maybe not. But a better approach is to improve the disc physically, to get it off of the nerve and to have it heal better than it will if it’s simply injected. I recommend injections in about 5% of patients, for whom spinal decompression is not sufficient to resolve the pain. But most of the time injections will not be necessary, and that saves a lot of money, and the potential risks associated with injections, which are modest, but real.
